Sometimes, processes can subtly change over time quite accidentally. If we all compared our driving skills from when we passed our test to now, we would probably see that a number of bad habits have crept in. The same can happen with how well we follow standard operating procedures (SOPs).

The processes we need to follow might also change over time because of a changing environment. Improving the capability of a team member or introducing someone new to the team may require us to rethink the processes that we use. Hands-on experience of using a particular SOP might help us to identify a more efficient, effective or safer way of carrying out a task.

SOPs should be reviewed at least annually to ensure they are being followed properly and that they are still current. SOPs for new processes should be reviewed more frequently as the quality of how well they have been embedded in needs to be reviewed.

Having others in the team follow ACPTs through preparation and regular reassessment could help improve dispensing skills and provide good evidence of competency that could form part of the risk management records for the pharmacy.
