Who can use AVARANTE?

AVARANTE is for men aged 18 years or over with ED. The recommended dose is one 10mg tablet taken with or without food, at least 30 minutes prior to anticipated sexual activity.13 AVARANTE can start to work in 30 minutes and may be effective for up to 36 hours after taking it. Sexual stimulation is required for AVARANTE to be effective.13

Dosing information

The maximum dosing frequency of OTC tadalafil 10mg is once per day.13 It should not be taken within 36 hours of a previous dose, and it is NOT recommended for continuous daily use.13 Men who use AVARANTE frequently (i.e. at least twice weekly) should be referred to their GP to discuss the suitability of a once-daily regimen with a lower dose,13 or consider a suitable alternative OTC PDE5 inhibitor that is suitable for daily use, if required (e.g. VIAGRA CONNECT).19

Using AVARANTE

The concomitant use of AVARANTE and other PDE5 inhibitors or other treatment for ED is not recommended.13 If required, customers may use different OTC PDE5 inhibitors at different times, as long as they continue to meet the relevant criteria for the supply from the pharmacy.6,20

Advise them to read the relevant patient information leaflet and encourage them to inform their doctor that they are obtaining treatment for their EP from the pharmacy.6

  • Customers already using tadalafil 10mg should maintain the 36 hour dosing frequency before starting AVARANTE6

  • Customers already using sildenafil 50mg should be advised to follow the correct dosing instructions before using AVARANTE6

Occasion-based usage

There may be instances when a customer could benefit from temporarily using one product instead of another. For example, a customer using VIAGRA CONNECT as their usual go-to treatment may choose to use AVARANTE when they plan a romantic weekend away.

NOTE: THESE TABLETS SHOULD NEVER BE TAKEN TOGETHER AT THE SAME TIME, OR ON THE SAME DAY. THE CUSTOMER WILL NEED TO WAIT AT LEAST 36 HOURS AFTER THEIR LAST AVARANTE DOSE BEFORE TAKING VIAGRA CONNECT ON THE NEXT OCCASION. CUSTOMERS TAKING VIAGRA CONNECT SHOULD WAIT 24 HOURS AFTER THEIR LAST DOSE BEFORE TAKING AVARANTE.

In summary:

IF A CUSTOMER IS USING MORE THAN ONE PDE5 INHIBITOR THEY SHOULD BE AWARE OF HOW LONG THEY NEED TO WAIT BETWEEN USING EACH PRODUCT.


If already using tadalafil 10mg(prescription or OTC)

Wait at least 36 hours after last dose before taking AVARANTE

If using sildenafil 50mg(prescription or OTC)

Wait at least 24 hours after last dose before taking AVARANTE


If using tadalafil 10mg(prescription or OTC)

Wait at least 36 hours after last dose before taking sildenafil 50mg

Assessing your customer’s suitability for supply6,13


FIRST TIME USERS

Once you have established that your customer has ED, follow steps 1 to 3 below to confirm whether it would be suitable to supply AVARANTE.

1. CHECK THAT YOUR CUSTOMER DOES NOT HAVE ANY CONTRAINDICATED HEALTH CONDITIONS

  • Myocardial infarction in the last 3 months

  • Unstable angina or angina during sex

  • Heart failure in the last 6 months of NYHA Class 2 or more (New York Heart Association (NYHA) heart failure classification: Class 2 heart failure is defined as slight limitation of physical activity, comfortable at rest, ordinary physical activity results in fatigue, palpitation, shortness of breath or chest pain)21

  • Arrhythmia, hypotension (<90/50 mmHg; symptoms of which include dizziness and blurred vision), uncontrolled hypertension

  • Stroke in the last 6 months

  • NAION (non-arteritic anterior ischaemic optic neuropathy) causing loss of vision in one eye

  • Allergy to the active ingredient, tadalafil, or any of the excipients, including lactose intolerance.22 Rare hereditary problems of galactose intolerance, total lactase deficiency or glucose-galactose malabsorption

  • Anatomical deformations of the penis:

    • Bent or curved penis (angulation)

    • Scar tissue on or below the surface of the penis (cavernosal fibrosis)

    • Peyronie’s disease

2. CHECK THAT YOUR CUSTOMER DOES NOT HAVE ANY CONCOMITANT HEALTH CONDITIONS THAT REPRESENT WARNINGS

  • Severe renal impairment

  • Severe hepatic impairment

  • Priapism or conditions that predispose to priapism:

    • Sickle cell anaemia

    • Multiple myeloma

    • Leukaemia

  • Pelvic surgery or radical non-nerve sparing prostatectomy (the effectiveness of tadalafil is unknown in this group of patients)

  • Hereditary degenerative retinal disorders (e.g. retinitis pigmentosa)

  • Increased susceptibility to vasodilators

  • Successful vascularisation, asymptomatic controlled hypertension or mild valvular disease

  • Uncontrolled hypertension, moderate to severe valvular disease, left ventricular dysfunction, hypertrophic obstructive and other cardiomyopathies, or significant arrythmias

  • Breathlessness or chest pain from light to moderate physical activity

If the customer has any of these health conditions, AVARANTE may not be suitable and a benefit-risk assessment by a doctor is required before use. They should be referred to a doctor for assessment of suitability.

Customers with some low-risk cardiovascular conditions may be suitable for AVARANTE, as long as their doctor has advised they can resume sexual activity, e.g. those with asymptomatic controlled hypertension, mild valvular disease or who have had successful coronary artery bypass grafting, stenting or angioplasty.

3. CHECK THAT YOUR CUSTOMER IS NOT TAKING ANY CONTRAINDICATED MEDICATIONS, OR MEDICATIONS THAT REPRESENT WARNINGS

  • Nitric oxide (NO) donors or nitrates used for angina (e.g. molsidamine, nicorandil, glyceryl trinitrate, isosorbide dinitrate) or as the recreational drug “poppers” (amyl nitrite)

  • Guanylate cyclase stimulators used for pulmonary hypertension (e.g. riociguat)

  • Alpha blockers used for benign prostatic hypertrophy or hypertension (e.g. doxazosin)

  • CYP1A2 substrates used for asthma or COPD (e.g. theophylline)

  • CYP3A4 inhibitors used for:

    • HIV infections (e.g. ritonavir, saquinavir)

    • Fungal infections (e.g. ketoconazole, itraconazole)

    • Bacterial infections (e.g. clarithromycin, erythromycin)

    • Stomach ulcers or oesophageal reflux disease (e.g. cimetidine)

    • High blood pressure (e.g. diltiazem, verapamil)

  • CYP3A4 inducers used for bacterial infection (e.g. rifampicin) or for epilepsy (e.g. phenobarbital, phenytoin and carbamazepine)

  • 5-alpha reductase inhibitors used for benign prostatic hyperplasia or male pattern hair loss (e.g. finasteride)

  • AVARANTE should not be taken with recreational drugs as this may lead to unpredictable and dangerous drug-drug interactions. This could result in dizziness, decrease in blood pressure, coma, stroke or heart attack, seizures and priapism. Recreational drugs include, but are not limited to:

    • Nitrous oxide

    • GHB (gamma-hydroxybutyrate)

    • GBL (gamma-butyrolactone)

    • Methamphetamine

    • MDMA/ecstasy (3,4-methylenedioxymethamphetamine)

    • Ketamine

    • Flunitrazepam (AKA rohypnol)

    • Cannabis

    • Opioids

    • Cocaine

    • Recreational nitrates such as “poppers” (e.g. amyl nitrite)

Patients taking interacting medication should be referred to their doctor for advice.

 

Advise customers that EPs could be a sign of another health condition, including heart disease, hypertension, diabetes or depression. All men who are supplied with AVARANTE should see their doctor at the earliest opportunity and no later than six months after being supplied with the product for a clinical review of potential underlying health conditions and risk factors associated with ED.13

If your customer is not suitable for AVARANTE, you should still refer them to their GP to assess whether their EP may be a symptom of another condition or to assess whether another medicine may be more suitable for them.6

If the supply of AVARANTE is deemed appropriate, you can also provide your customer with an optional AVARANTE Repeat Purchase Card that they can show when they next wish to purchase AVARANTE. It also contains a reminder to arrange a health check with their doctor.

REPEAT USERS

It is not necessary to repeat the same checks with a customer requesting a repeat supply of AVARANTE. They should have an AVARANTE Repeat Purchase Card with a pharmacy stamp, indicating that AVARANTE had previously been deemed suitable for them.6

You do need to determine whether AVARANTE remains suitable for them:

  • Check that the customer has not experienced any problems relating to AVARANTE

    • Ask about side effects, provide advice on how to manage common side effects or refer the customer to their doctor if required

  • Check that there are no changes in your customer's health conditions or other medications since the last supply

    • Ask about changes that represent contraindications or warnings (as you would for a first-time user). If there are changes, revisit the questions on the AVARANTE Checklist to ensure further supply would still be appropriate and refer the customer if necessary

If there have been no changes, you can continue to supply AVARANTE and remind your customer to arrange a follow up with his doctor within 6 months of first use.

REFERRALS

If it is not appropriate to supply AVARANTE for any reason, you can refer the customer to their GP and provide them with an optional AVARANTE Referral Card.

A PARTNER REQUESTING THE PRODUCT ON BEHALF OF THEIR AFFECTED MALE PARTNER SHOULD BE ASKED TO ENCOURAGE THE SUFFERER TO VISIT THE PHARMACY OR THEIR DOCTOR FOR ADDITIONAL ADVICE.6
