It is important to understand what NHS services are being delivered locally. These might be those delivered by GPs directly or through their practice nurse team or those provided through community hospitals or secondary care outreach services. Knowing what is already in place means that pharmacy services can then be designed to meet the needs generated by these other services in a way that complements them and does not compete. For example, a local secondary care service providing support for rapid discharge of stroke patients could be complemented by a pharmacy-led INR monitoring service or a full anticoagulant management service.
